Section I - General
To correctly complete this section, make the following entries in the
appropriate blocks (see Figure 4-3):
Block 1 - Enter the organization requesting the special
hauling
permit.
Block 2 - Enter the organization's home station.
Block 3 - Enter the estimated time-date group for the movement as
follows:
Enter the starting time-date group in block 3a.
Block 4 - Enter the vehicle's start point.
Block 5 - Enter the vehicle's destination.
Block 6 - Enter the estimated time of arrival at state lines; if no
state lines are crossed, enter "NA."
Block 7 -Enter in chronological order all interstate highways, US
highways, state roads, and streets to be crossed during the movement,
including routes to be used to and from rest areas, fuel stops, and
areas where vehicles will remain overnight.
Block 8 - Enter escort requirements, if any.
Section II - Vehicle and Load Data
To complete this section correctly, make the following entries in the
appropriate blocks (see Figure 4-3):
Block 9(b) - Enter tonnage classification of vehicles.
If unknown,
refer to TB 55-46-1.
Block 9(c) - Enter number of vehicles.
Prepare a separate DD Form
1266 for each type of equipment and load; two identical pieces of
equipment with different loads must have different DD Forms 1266.
Block 9(d) - Enter vehicle registration number(s). If there is more
than one piece of identical equipment with identical loads, enter
"SEE BLOCK 12" in block 9(d) and enter the appropriate registration
number for the equipment in block 12.
