Escort vehicles. When escort vehicles are required, you must ensure
that they are equipped with warning lights or driven with headlights
on. When required, the lead vehicle must carry a sign on the front
reading "WIDE LOAD FOLLOWS." Also, the rear escort vehicle must have
a sign on the back reading "WIDE LOAD AHEAD."
PART C - COMPLETE DD FORM 1266 (REQUEST FOR
A special hauling permit is a permit or authorization issued by state
highway departments or other issuing authorities.
This permit
authorizes convoy vehicles containing dangerous/hazardous material or
vehicles exceeding legal weight or dimension limitations to travel
over specified public highways.
When the dimensions and weight of
the shipment exceed the maximum allowable limits for the state that
you are operating in, complete DD Form 1266.
You must submit a
completed DD Form 1266 to the ITO if the combination of the vehicle
and load is oversized or overweight.
You must submit it in four
copies, with one copy added for each state to be crossed.
This
action will be discussed in Part C of this lesson. You must submit
this request to the ITO at least 15 days before the convoy departs.
When completing DD Form 1266, use the following information:
Type of equipment.
Gross weight, axle
loads,
and spacing,
height,
width,
and
length of vehicles.
Origin and destination of movement.
Date and time of movement.
Nature of cargo (within security limitations).
When completing DD Form 1266, also list only identical vehicles with
loads of uniform weight and dimensions on the same DD Form 1266. DD
Form 1266 is divided into two sections:
Section I - General.
Section II - Vehicle and Load Data.
Each section contains items of information that you must complete.
Each section and the items within each section are described below.
Refer to Figures 4-3 and 4-4 and note the completed items in each
section of the form.
