Items to Be Checked Prior to Release of Loaded Vehicle.
Each section contains items of information that you must complete.
Given below are descriptions of each section and the items that you
must complete in that section. Refer to Figures 4-6 and 4-7 and note
the items to be completed in each section of the form.
Origin/Destination
Under the heading "Origin," furnish information as follows:
Enter the carrier of the special hauling permit in Line 1.
Enter the name of the driver in Line 2.
Enter the date and hour in Line 3.
Enter the installation or activity in Line 4.
Enter the driver's state permit number in Line 5.
Enter the medical examiner's certificate and date in Line 6.
Vehicle
Under the heading "Vehicle," furnish information as follows:
Enter type of vehicle in Box 1.
Enter truck number in Box 2.
Enter trailer number in Box 3.
Enter yes or no for sleeping cab in Box 4.
Enter yes or no for valid lease in Box 5.
Enter ICC number in Box 6.
NOTE:
In the middle section, items 1 through 22 are items that
you must check before loading the vehicle with hazardous
material.
If any of these items are unsatisfactory, you
must reject the vehicle.
Items with an asterisk (*) must
be checked on incoming loaded equipment.
